This liquid, sold as codeine and promethazine ("Lean"), does not contain codeine but diphenhydramine.
Diphenhydramine is a drug that is used in medicine. It is often used as a sleeping pill or to treat nausea. The substance has a calming and sedative effect. In higher doses, it can lead to confusion, hallucinations, delirium, seizures and cardiac arrhythmia.
Users of lean primarily report a sedative and euphoric effect. In contrast, users of diphenhydramine typically also report unpleasant effects such as restlessness, tension and paranoia. This difference in effect can lead to overdosing and thus to an unintentionally high dose of diphenhydramine being consumed.
Drugs from non-medical sources are often misdeclared - therefore: it is essential to have substances tested.
If drug checking is not available, start with a very low dose when using for the first time and look out for warning signs such as unexpected effects.
Mixed use with stimulants or other depressant substances is not recommended, as it increases the likelihood of undesirable or even life-threatening interactions.
Especially in combination with other sedating substances (e.g. alcohol, benzodiazepines, opioids), there is an increased risk of respiratory depression and impaired consciousness.

Safer Use
Analysis
Have your substance tested in a drug check if you bought it on the black market or on the internet. Counterfeit blisters may look identical to the original packaging.
Mixed use
Do not mix any depressant substances such as benzodiazepines, alcohol, GHB/GBL or opioids. Risk of unconsciousness or respiratory paralysis!
Dosage
Feel your way to the dosage and do not rely on dosage information from friends. Various factors such as habituation or dependence can contribute to a greatly increased dosage!
Testing
If you do not have the possibility to have substances analyzed, first test a maximum of one third of the pill. Some counterfeit drugs contain a more potent substance than the original.
Wait and see
Wait at least 2 hours after taking the drug. With some drugs, the onset of the intoxicating effect is very slow or it is difficult to perceive.
