These substances, known as cocaine , contains only a small amount of cocaine and consists mainly of caffeineand lidocaine and the synthetic stimulant 4-CMC.

Caffeine and 4-CMC stimulate the central nervous system and, when combined with cocaine, increase the strain on the heart, circulation, and psyche. Possible consequences include rapid heartbeat, restlessness, anxiety, and overheating. There is also a risk of consuming large amounts of cutting agents if the desired cocaine effect is not achieved and consumption is therefore continued.

Lidocaine is a local anesthetic that inhibits and numbs nerves when applied externally. It is regularly used as a cutting agent in cocaine samples. The numbing effect can be mistakenly perceived as an indication of "strong" or "pure" cocaine. Lidocaine is particularly dangerous when used intravenously, as it can cause severe disturbances in cardiac conduction, a drop in blood pressure, numbness, and allergic reactions.

4-CMC belongs to the group of cathinones and is structurally related to mephedrone (4-MMC). Its effects differ from those of cocaine and are described as highly performance-enhancing and euphoric. Some users report severe headaches and kidney or liver pain in the days following consumption. In addition, a pronounced urge to take more is described, which increases the potential for dependence. 4-CMC is suspected of being highly neurotoxic; only limited information is available about other risks, side effects, and possible long-term consequences.

Due to the potentially highly neurotoxic effects of 4-CMC and the indeterminable health risks, consumption of this substance is not recommended.
